Enrollment
Below are all the required forms to finalize your child’s enrollment at Magic of Play Preschool. These forms are only for families with a secured spot in our program. They are NOT needed to request registration.

Child Information Record
This form is required by the state and provides us with basic information about your child.

Health Appraisal
This form is required by the state and must be filled out and signed by a licensed physician. If your child is under 2.5 years, the form must be based on an exam within 6 months preceding the first day of school. If your child is 2.5 years or older, the form must be based on an exam within 12 months preceding the first day of school. For all students, a copy of your child’s immunization record must also be included.

Agreements & Consent Form
This form is unique to Magic of Play Preschool and is based on licensing requirements. It includes consent for topical application, acknowledgment of the Family Handbook, property exploration, and media use.

Acknowledgment of Outdoor Education Risks
There are many benefits to learning and playing outside, but there are some risks involved. Please read and sign this document to accept and acknowledge the risks of outdoor education.
Annual Calendar
Our 2025-2026 calendar runs from November–early June. We close for typical school breaks and do not have school on Fridays. Our start date for this year is tentative until we are certain when construction will end. If the start of school is delayed due to building construction, we will discount tuition accordingly.
